What to Expect from the Program…

The staff will give you a set of individual guidelines to help answer all your questions concerning:

    • Therapy and meeting schedules
    • Meals
    • Visiting hours
    • Therapeutic passes
    • Clothing needs
    • Telephone needs
  • Daily Schedule…

Each day, you will participate in physical and occupational therapies. A variety of other treatments may be provided, such as speech therapy, psychological rehabilitation activities and leisure pursuits. Each session is structured around your specific needs.

You will learn how to be independent and how to adjust to the physical limitations you are experiencing. By learning about your condition, talking about your experiences, and listening to fellow patients, you’ll discover how to live a productive life with your disability.

  • Staff Meetings -- Once a week, the rehab team will meet to review your progress; they will discuss the outcome of those meetings with you.
  • Family Conferences – Every patient is offered at least one family conference to address patient goals, progress made and discharge plans. Staff from each discipline will discuss your rehab plan personally.
  • Meals – Meals are held in the program’s dining room. You are encouraged to join the others at meal times to enjoy your food in a friendly, social atmosphere.
  • Therapeutic Passes – A temporary leave from your rehab program may provide you and your family with an opportunity to practice new skills outside of the hospital setting. A pass is typically scheduled on a weekend day just prior to discharge from the program.
  • Case coordination and treatment planning

A staff member of the program will be your case coordinator who will make sure you participate in the activities that you and the team have determined are best suited for you. Your case coordinator will also help you participate in the planning of your treatment. You will need to tell us your goals and how you would like to achieve them while you are here.

Our goal is to help you learn the skills you need to care for yourself. In addition, throughout your rehabilitation, family conferences may be held with the Rehabilitation Center team to review your progress, rehabilitation goals and discharge plans. Working together, we will succeed.

We want you to be free from worry during your stay. Our staff members will gladly answer any questions about your therapy at The Rehabilitation Center at Deaconess Hospital.
