The Progressive Care Center

The Progressive Care Center at Deaconess bridges the gap between a patient’s hospital stay and his or her eventual return to a more independent lifestyle at home or a nursing home. It is a comprehensive hospital-based medical and subacute physical rehabilitation program providing integrated and coordinated therapeutic service and treatments to those in need of transitional care following an acute hospitalization for an illness, injury or surgery. A free pre-admission screening will be conducted prior to admission.

Admission to the Progressive Care Center requires referral from a physician, and a three-day qualifying stay in an acute care hospital. In addition, you are expected to exhibit continuous improvement, progress or potential benefit from the plan of care developed upon admission. Criteria for admission will be applied equally to all who are referred for services regardless of funding source.
